Commercial Driver Medical Certification in Ontario
Becoming a professional driver in Ontario requires several steps, one of which is the commercial driver medical certification. Understanding and meeting the Ministry of Ontario's established medical, vision, and hearing standards for commercial drivers is essential.
Commercial drivers (Classes A, B, C, D, E, and F) must submit a medical report when applying for or renewing a license. They are held to higher licensing standards because they drive longer hours, often in adverse weather conditions, and face greater physical demands due to operating large, heavy vehicles.
Drivers with Class A, B, C, D, E, and F licenses must submit additional medical reports for renewing their commercial driver’s license:
- Every five years, if under 46 years old
- Every three years, if between 46 and 64 years old
- Annually, if 65 years old or older
The ministry will send the driver's medical forms 90 days before the due date. This will give you enough time to schedule an appointment with your physician or nurse practitioner and return the completed form by the due date. The form will be mailed to the address listed on your driver’s license. These requirements align with the recommendations of the Canadian Council of Motor Transport Administrators, which are the basis of the U.S./Canadian Reciprocal Agreement for commercial drivers crossing the Canada/U.S. border.
What to Expect During the Driver's Medical Exam
When undergoing a commercial driver's medical exam, here are some of the tests that you may be expected to undergo:
- Assessment and review of the patient's medical history
- Urine testing
- Vision testing
- Hearing test
- Filling out medical forms
- Conducting any other medical tests listed on the MTO medical form for commercial drivers
Submitting Your Commercial Driver's Medical Forms
You must submit a medical report if you are applying for or renewing a class A, B, C, D, E or F licence. The medical report form can be completed by a physician or a nurse practitioner (RN-EC) licensed to practice medicine in Canada. The medical form can be obtained from your local DriveTest centre if you are a first-time applicant, or you will receive a medical report form by mail 90 days before it is due if you are renewing your license.

Send the completed form along with any necessary supporting documents to the address specified on the form for the Ministry of Transportation, Transportation Safety, Driver and Vehicle Services, Driver Medical Review Office.

If your physician or nurse practitioner has access to eForms at their clinic, they can submit your completed medical report form to the Ministry of Transportation’s Driver Medical Review office online.
